Daily Symptoms Checklist for Parents/Guardians

 

In an effort to maximize the health and safety of the Needham Public Schools community, please perform a daily wellness check on your student each day before he/she/they leave for school and attest to their health. You are being asked to consider symptoms whether your student is vaccinated or unvaccinated. If your child is experiencing the specified symptoms, please keep them home from school and all extracurricular activities and call their healthcare provider and school nurse.  

The district procured Ruvna which uses text messaging and emails to push out a web link each morning, allowing families to quickly tap their way through health screening questions for the students in their household. Please reference this Q& A for additional information about the health screening process and the Ruvna app.

 

If your child is fully vaccinated: In the past 24 hours, has your child had any of the following symptoms?  If yes, please keep them home.

- Fever (temperature over 100℉), chills, or shaking

- Difficulty breathing or shortness of breath

- New loss of taste or smell

- Muscle aches or body aches

- Cough (not due to other known cause, such as chronic cough)

 

If your child is unvaccinated or partially vaccinated: In the past 24 hours, has your child had any of the following symptoms? If yes, please keep your unvaccinated or partially vaccinated child home.

- Fever (temperature over 100℉), chills, or shaking

- Difficulty breathing or shortness of breath

- New loss of taste or smell

- Muscle aches or body aches

- Cough (not due to other known cause, such as chronic cough)

- Sore throat when in combination with other symptoms

- Nausea, vomiting, or diarrhea when in combination with other symptoms

- Fatigue, when in combination with other symptoms

- Nasal congestion or runny nose (not due to other known causes, such as allergies), when in combination with other symptoms

- Headache, when in combination with other symptoms

 

Please answer the following questions; if the answer is yes, please keep your student home and call the school nurse:

 

- Have you or anyone in your family received a positive test result for COVID-19 in the past 14 days?  If yes, you must notify the school nurse and keep your child home.

 

- In the past 14 days, has your child been identified as a close contact to an individual diagnosed with COVID-19?  If yes, you must notify the school nurse.

 

- Is your child waiting for a test result for COVID-19? If yes, you must notify the school nurse and keep your child home.

 

- Has your child been out of state for greater than 24 hours in the past 10 days?  If yes, please follow the Needham Public Schools Travel Advisory.

Thank you for your cooperation; it is extremely important that you complete this wellness check on each of your children, each day before school, in order to reduce the risk of COVID-19 spread in our community.
